In order to complete your registration with Locum Link and also comply with the Code of Practice we require the following:
- Completed Registration Form and Health Questionnaire
- Bank Details
- A copy of your most recent CV (with details of any gaps of employment)
- Names of at least two of your most recent referees
- A copy of your GMC Certificate
- Proof of Hepatitis B IVS with titre levels
- Occupational Health/Laboratory reports with details of Varicella, TB & Rubella
- 2 passport size photographs
- National Insurance Number
- Copy of membership with a professional indemnity organisation
- A copy of your passport and birth certificate
- A copy of your Eligibility to work in the UK (Visa), with supporting documentation
